Blend’s accident, sickness and travel insurance claims are handled by a specialist panel of third-party claims providers.


Our panel have significant industry experience and work closely with both us and Allied World to ensure all claims are handled in an expert manner.

The nominated claims handler will depend on the type of Blend policy you hold and the corresponding PDS code:

For Blend policies with the following PDS codes: 

  • Blend.IPA.1124 & 0525
  • Blend.VNW.1124 & 0525
  • Blend.BTA.1124 & 0525
  • Blend.GPA.1124 & 0525
  • Blend.SPA.1124 & 0525
  • Blend.GPA.IFS.1124 & 0525
  • Blend.GPA.LIA.1124 & 0525

If your policy start date is before 1 May 2025 the claim will be notified to and managed by Corporate Services Network

If your policy start date is on or after 1 May 2025 the claim will be notified to and managed by Proclaim


For Blend policies with the following PDS codes: 

  • Blend.GPA.Chifley.1124 & 0525
  • Blend.GPA2.Chifley.1124 & 0525


The claim will be notified to and managed by Employers Mutual Limited



How to notify a claim:

You must provide notification in writing within thirty (30) days of an event that is likely to give rise to a claim. 
If it is not possible to notify within that time, you must notify as soon as reasonably possible. 
Once notified of a claim, the relevant claims handler will provide you with a claim reference. 
They will also outline what you need to provide in support of your claim to be considered.

You must fully complete and return the claim form (as relevant) and provide the supporting documentation to
them. This information can include health certificates, medical practitioners’ reports, employer reports and related evidence of the claim.

  1. New Personal Accident Claim Checklist
  2. Personal Accident & Sickness Claim Form (to be used for Individual & Group Personal Accident & Sickness policies)
  3. TFN Declaration Form to attach together with completed claim form with new claim notification, and
  4. Travel Insurnace Claim Form (to be used for Business Travel Insurance policies).
